    目的? 探讨不同性别和不同年龄的呼吸道感染儿童 25 羟 - 维生素 D[25(OH)D] 的浓度变化,并分析 25(OH)D 与免疫球蛋白的相关性。方法? 选取 2019 年 6 月至 2020 年 2 月绵阳市中心医院就诊的呼吸道感染儿童 280 例,其按年龄分为新生儿组、婴儿组、幼儿组和学龄前儿童组;按 25(OH)D 的浓度分为 25(OH)D 缺乏、25(OH)D 不足、25(OH)D 充足和 25(OH)D 过量。采用高效液相色谱串联质谱法测定血清 25(OH)D 的浓度,散射比浊免疫法测定血清免疫球蛋白的浓度。结果? 不同年龄阶段的呼吸道感染儿童 25(OH)D 的浓度有显著性差异,各年龄组儿童 25(OH)D 过量、充足、不足和缺乏的构成不同,差异有统计学意义(P < 0.01)。不同性别的呼吸道感染儿童 25(OH)D 的浓度差异有统计学意义(P < 0.01),呼吸道感染儿童中,不同 25(OH)D 浓度的构成比,免疫球蛋白的浓度有显著性差异,25(OH)D 的浓度与免疫球蛋白 A(IgA)、免疫球蛋白 M(IgM)和免疫球蛋白 E(IgE)有相关关系(r 分别为 0.424、0.496 和 0.465,P 值分别为 0.000、0.000 和 0.000)。结论? 呼吸道感染儿童 25(OH)D 与免疫球蛋白有明显相关关系,呼吸道感染儿童补充维生素 D 有助于提高免疫功能,增强免疫力。

    Abstract:

    Objective? To investigate the change of 25 hydroxy-vitamin D[25(OH)D] concentration in children with respiratory tract infection of different sex and age,and analyze the correlation between 25(OH)D and immunoglobulin. Methods? A total of 280 children with respiratory tract infection in the Mianyang Central Hospital from June 2019 to February 2020 were included and divided into neonatal group,infant group,infant group and preschool group by age. The concentration of 25(OH)D was divided into 25(OH)D deficiency,25(OH)D,25(OH)D adequacy and 25(OH)D excess. High performance liquid chromatography tandem. Concentration of serum 25(OH)D was determined by mass spectrometry and serum immunoglobulin was determined by scattering turbidimetric immunoassay. Results? There was significant difference in the concentration of 25(OH)D in children with respiratory tract infection at different ages,and the composition of 25(OH)D excess,adequacy,deficiency and deficiency in children of different age groups was different,the difference was statistically significant(P<0.01). The concentration of 25(OH)D in children with respiratory tract infection of different sex was statistically significant(P<0.01),and the composition ratio of 25(OH)D concentration in children with respiratory tract infection was significantly different. The concentration of immunoglobulin was significantly different. The concentration of 25(OH)D was correlated with IgA,IgM and IgE(r=0.424,0.496 and 0.465,P=0.000,0.000 and 0.000,respectively). Conclusion? 25(OH)D in children with respiratory tract infection is associated with immunoglobulin. Vitamin supplements for children with respiratory infections D help improve immune function,enhance immunity.
