    目的?探讨输尿管软镜碎石取石?(RIRS)术后辅助物理振动排石?(EPVL)的时间点选择对排净软镜术后肾内残石的疗效。方?法 选取郑州大学第二附属医院 2018 年 6月至 2019 年 12月采用输尿管软镜碎石取石治疗的 180 例上尿路结石患者为研究对象,按照随机数字表法分组选择不同的治疗时间点分为试验 A 组、试验 B 组、对照组,每组 60 例。所有患者均行输尿管软镜碎石取石治疗并于术后1?d复查影像学提示有肾内残石堆积,其中试验 A组于术后第3天行EPVL,试验 B组于术后第10天行EPVL,对照组行传统自然排石法。所有患者均在术后 2、4 周复查彩超或 CT,比较各组的结石清除率及并不良反应发生率。结果 术后 2 周,试验 A 组清石率?(SFR)显著高于试验 B 组与对照组。术后 4 周,试验 A 组 SFR 最高,与试验 B 组比较,差异无统计学意义?(P > 0.05),试验 A 组与试验 B 组 SFR 均高于对照组?(P < 0.05)。术后试验 A 组、试验 B 组不良反应发生率低于对照组?(P < 0.05)。结论 输尿管软镜碎石取石术后辅助物理振动排石清石率高且不良反应少。

    Abstract:

    Objective?To investigate the efficacy of the timing of assisted physical vibration lithotripsy(EPVL)after ureteroscopy for the removal of intrarenal residual stones(RIRS)after flexible lithotripsy. Methods?A total of 180 patients with upper urinary tract stones treated by ureteroscopy from June 2018 to December 2019 at the Second Affiliated Hospital of Zhengzhou University were retrospectively enrolled,and divided into group A,group B,and the control group,with 60 patients in each group. All patients underwent ureteroscopic treatment and re examination of the imaging 1 d after surgery suggested intrarenal residual stone accumulation,in which group A underwent EPVL on postoperative day 3,group B underwent EPVL on postoperative day 10,and the control group underwent conventional natural lithotripsy. All patients were reviewed by color ultrasound or CT at 2 and 4 weeks after operation to compare the stone clearance rate and the incidence of complications among groups. Results?Two weeks postoperatively,the stone free rate(SFR)in group A was significantly higher than that in group B and control group. At 4 weeks postoperatively,group A had the highest SFR,which was not statistically significant compared with group B(P >0.05).The SFR of group A and group B was significantly higher than control group. The incidence of postoperative adverse effects of group A and group B was lower than in the control group(P < 0.05). Conclusion?Ureteroscopy assisted by physical vibration lithotripsy can be used as early as day 3 after RIRS and is safe with few complications.
