    目的? 探讨外固定支架牵引复位结合微创内固定系统(LISS)钢板治疗胫骨骨折对患者血液流变学的影响与安全性。方 法? 回顾性分析徐州市中心医院 2015 年 6 月至 2019 年 7 月行手术治疗的 83 例单侧闭合性胫骨骨折患者的临床资料,按手术方式的不同分为 A 组(40 例,行常规切开复位钢板内固定术治疗)和 B 组(43 例,行术中外固定支架牵引复位结合 LISS 钢板治疗),两组患者均于术后随访 9 个月。比较两组患者术后 9 个月临床疗效,术前、术后 1 个月血液流变学指标水平,术后并发症发生情况。结果? 术后 9 个月 B 组患者临床总有效率高于 A 组;与术前比,术后 1 个月两组患者血浆黏度(PV)、低切全血黏度、高切全血黏度水平均降低,且 B 组低于 A 组;B 组患者术后并发症总发生率低于 A 组(均P<0.05)。结论? 外固定支架牵引复位结合 LISS 钢板治疗胫骨骨折可提高患者临床疗效,并改善患者血液流变学,且安全性较好。

    Abstract:

    Objective?To explore the effect and safety of external fixation traction reduction combined with minimally Less Invasive Stabilization System (LISS) stell plate for tibial fractures on patients’ hemorheology. Methods?The clinical data of 83 patients with unilateral closed tibial fracture who underwent surgical treatment in Xuzhou Central Hospital from June 2015 to July 2019 were retrospectively analyzed, according to the different surgical methods, they were divided into group A (40 cases, underwent conventional open reduction and internal fixation with steel plate) and group B (43 cases, underwent intraoperative traction reduction with external fixation stent combined with LISS steel plate treatment), after the operation, patients in both groups were followed up for 9 months. The clinical efficacy at 9 months after operation, hemorheology index levels before and 1 month after operation and the occurrence of postoperative complications were compared between the two groups. Results?The total clinical effective rate of patients in group B was higher than that in group A at 9 months after operation; compared with preoperatively, the plasma viscosity (PV), low-cut whole blood viscosity and high-cut whole blood viscosity levels of two groups of patients decreased at 1 month after operation, and group B was lower than group A; the total incidence of postoperative complications in group B was lower than that in group A(all P<0.05). Conclusion?The treatment of tibial fractures with external fixator traction and reduction combined with LISS steel plate can improve the clinical efficacy of patients and improve the hemorheology of patients, and the safety is better.
