    目的 研究上海市松江区健康体检人群的肿瘤标记物(TM)水平及其应用价值。方法 选取 2021 年 1 月至 12 月到上海市 松江区中心医院体检中心进行健康体检的 37 695 例体检者为研究对象进行回顾性分析,分析不同 TM 的检测水平及其异常检出率,比较 不同性别、不同年龄体检人群的 TM 水平。结果 检测结果显示,所有 TM 中细胞角蛋白 19 片段(CY211)异常检出率最高,男性癌胚 抗原(CEA)和 CY211 异常率明显高于女性(P<0.05),而女性糖类抗原 199(CA199)异常率高于男性(P<0.05),不同性别人群中甲 胎蛋白(AFP)异常检出率比较,差异无统计学意义(P>0.05)。AFP、CEA、CA199、CY211 均在 61 岁以上年龄组体检者中异常检出 率最高,男性人群总前列腺特异性抗原(TPSA)、游离前列腺特异性抗原(FPSA)及女性人群糖类抗原 153(CA153)也是在 61 岁以 上年龄组体检者中异常检出率最高。CEA 和男性 TPSA 异常检出率随年龄增加而升高。女性糖类抗原 125(CA125)在 31~40 岁的年龄组 异常检出率最高。结论 不同性别、不同年龄的体检人群 TM 水平比较,差异有统计学意义,联合检测多项 TM 有利于疾病的早发现早 治疗。

    Abstract:

    Objective To study the level and application value of tumor marker (TM) in healthy population in Songjiang district of Shanghai. Methods A total of 37 695 healthy people from Songjiang District who have done physical examination in the physical examination center of Shanghai Songjiang District Central Hospital from January 2021 to December 2021 were included in this study. This study was a retrospective data analysis. Different TM detection levels and abnormal detection rates were analyzed, and the TM levels of people with different genders and different ages were compared. Results The results showed that the abnormal detection rate of cytokeratin 19 fragment (CY211) in all TM was the highest, and the abnormal rate of carcinoembryonic antigen (CEA) and CY211 in males was significantly higher than that in females (P<0.05), while the abnormal detection rate of carbohydrate antigen 199(CA199) in females was higher than that in males (P<0.05). There was no significant difference in the abnormal detection rate of alpha fetoprotein (AFP) in different gender groups (P>0.05). The abnormal detection rates of AFP, CEA, CA199 and CY211 were the highest in the age group over 61 years. The abnormal detection rates of total prostate specific antigen (TPSA), free prostate specific antigen (FPSA) and carbohydrate antigen 153(CA153) in the male population were also the highest in the age group over 61 years. The abnormal detection rates of CEA and male TPSA increased with age. The abnormal detection rate of carbohydrate antigen 125(CA125) in female population was the highest in the age group of 31~40. Conclusion There are significant differences in the levels of TM among different gender and age groups. Combined detection of multiple TM is beneficial to early detection and treatment of the disease.
